Position Summary:

Reporting to the Clinical Manager of Pharmacy Services and under leadership of the Manager of Enabling Patient Access & Patient Flow, you will work collaboratively as a part of an integral member of the Integrated Transition team and be accountable for the pharmacotherapy for patients to ensure optimal patient care is achieved. You will be evaluating evidence-based medicine to support your recommendations and to develop improvement strategies for drug utilization.

Key Qualifications:

· Baccalaureate Degree in Pharmacy

· Registered as a Pharmacist (Part A) with the Ontario College of Pharmacists

· Doctor of Pharmacy (PharmD) is an asset

· Membership with the Canadian Society of Hospital Pharmacists is an asset

· 3-5 years of experience in a hospital setting that may include a completion of hospital pharmacy residency or equivalent experience

· Relevant experience in managing acute patients in a variety of programs: orthopaedic, neurological, medical, surgical, obstetrics care, psychiatric care, critical care, pediatrics, geriatrics, community health, occupational health, emergency care, rehabilitation or oncology and/or rehab adult populations surgery,

· Effective interpersonal skills combined with the ability to work in collaboration with all members of the Health Care Team both internally and externally across the continuum of care

· Strong team building, problem solving, organizational and communication skills

· Experience in providing exceptional patient centered care

· Experience in patient medication counselling, hospital pharmacy systems and distribution systems are an asset

· Demonstrated research skills and knowledge of community health programs is a definite asset

· The ability to function effectively in a fast paced environment during periods of rapid change and transition

· Demonstrated the ability to work on a dynamic, interprofessional team

· Demonstrated excellent organizational skills and is able to be self-directed

· Skills in program development an asset

· Valid Ontario Driver"s License, relevant insurance and use of own vehicle

Responsibilities Include but are not limited to:

· Provides recommendations for optimal pharmacologic outcomes and prescribing during consultations, rounds or as part of the drug utilization and evaluation process

· Supports and leads knowledge transfer and utilization of best practices for medication utilization and strategies to enhance patient outcomes in older adults

· Supports the ongoing development of systems to enhance the ability of patients to safely and effectively transition between areas of the hospital and upon discharge.

· Provides a structured clinical pharmacy service that promotes optimal pharmaceutical care for patients

· Prevents, identifies and resolves drug-related problems

· Educates clients and/or their caregivers on proper medication use to ensure optimal medication taking behaviour and resolves drug therapy problems as they arise

· Documents consultation reports in an efficient manner and communicates outcomes to key stakeholders and members of the interprofessional team.

· Liaises with family physicians and other community health professionals as needed

· Contributes to support the development of a transitional team

· Provision of pharmaceutical care to patients through a consult service or as part of the transition team function.

· Clinical care of patients in comprehensive programs (i.e. IV to PO, Aminoglycoside dosing, Vancomycin dosing, Renal dosing, Pharmacist-Assisted Warfarin Service, Medication Reconciliation)

· In collaboration with other clinical pharmacists will deliver appropriate teaching to pharmacy residents and pharmacy students

· Participation in pharmacy case presentations, Pharmacy Education Rounds, journal clubs and case discussions

· Drug distribution responsibilities.

· Participates in drug use evaluation and drug order review

· Drug therapy monitoring

· Supervision of Pharmacy Technicians as required
